- Admit Card [Madhyamik Exam] for verification of age
- ID & Address proof (Adhar Card/Voter Card)
- A Medical Certificate of Fitness from a registered Medical Practitioner(Original)
- Character Certificate [1Copy] current as far as practicable (Original)
- Valid Cast Certificate, if allotted category in either ST/SC/OBC(A)/OBC(B) and EWS only
- Valid Disability Certificate for PH candidates with locomotor disorder only approved by Faculty
- Passport size photograph (2 copies)
